Application Fee Waiver Request Form

Applicants to the MS or PhD programs in mathematics meeting all the following criteria can receive a waiver of the normal application fee.
Note that we cannot waive your fee until your application is submitted, with all required supplementary materials already received (i.e. fully complete except for payment of the application fee).
1) Your application is complete (based on your application checklist except the application fee) by February 1st.
2) You have an overall GPA of 3.0
3) You request a reimbursement by submitting this form.
